When you enrol with us, some of the benefits include:
Reminders and recalls for important health management
Pay less to see the doctor than non-enrolled patients
Access to our online patient portal to book appointment, order repeat prescriptions and to view lab test results
Unfortunately we are unable to accept Casual enrolments
Enrol Online
There’s no need to come into the clinic, you can enrol online - it’s secure and confidential. By completing the online form it is understood that you agree with the Health Information and Privacy Statement and our Terms of Trade. The online form will guide you and you will need to upload suitable identification. (Please note we are unable to accept casual enrolments).
Paper Enrolment
Paper enrolment forms are still available if you're not able to complete the online form. Simply print and complete a form for each person enrolling, ensuring they have also read the Health Information and Privacy Statement. Then email all files to reception@albanydocs.co.nz including a copy of your suitable identification.
Terms of trade
When you enrol with us, it is understood that you agree to our Terms of Trade.
We require that all accounts are paid in full on the day of your consultation (including phone and email consults) unless a prior payment agreement is already in place.
If any balances of 90 days or more are not paid, it is understood that these will be forwarded onto Baycorp and the client will be liable for any collection costs.
